Forgot to complete the questionaire

Dear Sirs,

I have completed the course 幼教101 幼兒成長與發展 by end of July but forgot to complete the questionaire at last.

Could you please kindly help to issue the certificate to me? I cannot register online and complete the questionaire.

我看過你的帳戶資料。您應該於7月19日已經完成10年第1季101課程所有課程內容了 (完成課程的最後限期為2010年7月31日)，只是忘了填寫證書問卷的資料，對不對？

我補發一張證書給您吧。下次記得填網上問卷了！(我會請同事明天寄出。下星期前應該可以收到的。)
